PTO Funds Request Form - (New Single Form for General and Teacher Fund Requests)
  • For PTO General Fund requests, please check off the box requesting General Funds.  Fill out the form completely and submit to Mr. Blackburn for review.    If approved by Mr. Blackburn, the PTO board will then review the request and decide if it is appropriate and allow a vote at the next PTO meeting.   Thank You.
  • For PTO Teacher Fund requests, please check off the box requesting Teacher Funds.  The Teacher Funds are accumulated through Market Day/Santa Breakfast baskets and used for classroom items.   Fill out the form, attach your receipts to the form and place the form in the PTO folder in the main office.  Please email the PTO Board at to notify us that a request is in the folder.  If you do not have a sufficient amount in your fund, we will reimburse you up to the amount available   When you receive your reimbursement check, we will also record the amount left in your fund.   Thank You.
